Overview
Grace Heights Health and Rehabilitation is seeking a RN or LPN to become a first shift supervisor for our 120-bed facility. This position will provide clinical and organizational support to our Director of Nursing and leadership to the direct care staff.
Qualifications
The ideal candidate will have
at least one year of supervisory experience in long-term care , be compassionate and caring, handle multiple priorities, work flexible hours and motivate staff.
RN or LPN license required.
Direct experience with Electronic Medical Record software - Matrix a plus.
Responsibilities
Provides and helps to coordinate clinical services for residents in the nursing facility
Facilitates in the resolution of issues and concerns associated with resident/patient care including family issues
Supports the achievement of deficiency-free status by monitoring compliance with requirements for Medicare/Medicaid recipients; reporting on compliance
on-compliance issues
Helps to ensure that staff are properly trained, oriented, and regularly in-serviced
Ensures frequent communication with residents and their loved ones to keep them informed and involved in their care
Assesses and evaluates the systems which facilitate the delivery of quality resident care
Supports the fulfillment of Camden's continuing care philosophy and the maintenance of long-term care certification by adhering to professional standards, organization policies and procedures, compliance with federal, state, and local regulations
Direct experience with Electronic Medical Record software - Matrix a plus
Implements and evaluates all nursing procedures and systems on the units
Completes required forms and documents in accordance with company policy and state and/or federal regulations
Ensures continuity of care and discharge planning by coordinating the transmission of needed information and by attending discharge planning and care plan meetings
Communicates with the Admissions Coordinator to input new admissions into the system and assists in the discharge process
Will perform general nursing duties in cases of emergency to ensure proper staffing coverage
May be required to work a med cart
All other duties as assigned
