Intensive Case Manager, Home Study and Post Release Services

Overview
The Intensive Case Manager provides focused, collaborative home study services to unaccompanied children and their sponsors to ensure safe and timely release from federal care. In addition, the HSPRS Caseworker applies clinical expertise in therapeutic, strengths‑based case management for children requiring Level 3 services upon reunification. Weekly intensive in‑home services are required.

Responsibilities

Home Study

Conduct in‑person home visits to interview, assess, recognize priorities, and trends to inform psycho‑educational sessions with sponsors.

Complete a home‑study recommendation within 10 days of referral.

Post Release Services

Conduct weekly in‑person home visits with children released from federal care to stabilize placement and promote safety, assessing risk and protective factors.

Conduct safety planning and risk assessment, including crisis intervention, focusing on family preservation and collaboration with behavioral health providers.

Advocate for the child and sponsor family to address barriers to accessing resources and navigate complex systems such as child welfare and immigration court.

Conduct child‑centered needs assessments.

Develop and prepare individualized, goal‑oriented service plans.

Coordinate referrals and address barriers in legal, education, medical, mental health areas; support children and sponsors in enrollment and access to community services.

Coordinate access to youth programming and activities that promote cultural connection.

Provide psycho‑education to children and sponsors on safety topics.

Apply clinical insights in designing supportive plans for families coping with reunification stressors.

Monitor progress toward goal completion.

Monitor safety and well‑being of children, including indications of trafficking, abuse, and neglect; create and revise safety plans as needed.

Serve as a mandated reporter of child abuse and neglect and work with state child welfare as needed.

Network and develop partnerships within the community to educate providers on children’s needs and reinforce collaboration in relevant areas.

Prepare case notes, enter data, and complete reports promptly.

Coordinate internally with supervisors and case workers to accept referrals of children receiving Level 1 or 2 services that could benefit from more intensive Level 3 services.

Participate in supervision, team meetings, and trainings.

Other duties as assigned.

Qualifications

Master’s degree in social work, psychology, sociology, or another relevant behavioral science with direct clinical experience required.

Consideration given to candidates holding a bachelor’s degree with at least five years of demonstrated clinical case management experience.

Experience

MSW candidates must have at least three years of experience (five years if holding a BSW) working with children and youth, family reunification, or child welfare/foster care. Prior experience with foreign‑born populations required; ability to provide trauma‑informed assessment, crisis intervention, safety planning.

Prior experience identifying community social services and resources.

Experience navigating complex systems including child welfare, immigration, court systems.

Prior clinical experience working with clients in acute crisis.

Licensure or eligibility for licensure as required by state.

Minimum of two years of post‑graduate direct clinical service experience.

Skills

Ability to read and write in Spanish and English.

Ability to establish trust and rapport with clients.

Ability to maintain calm and use sound judgment in high‑stress situations.

Special Requirements

Must possess and maintain active licensure or be eligible for licensure.

Incumbent must be located in Florida, Indiana, or Texas.

Fluency in English and Spanish.

Valid driver’s license and insurance; personal vehicle required.

Comfortable conducting visits in client’s home, including unfamiliar areas; willing to travel frequently and with limited notice.

Comfortable conducting home visits in diverse socio‑economic communities.

Availability to work evenings and weekends as assigned.

Must successfully pass PSA, FBI criminal, and Child Abuse and Neglect Checks.
