DESCRIPTION OF WORK
This professional position is responsible for providing case management to individuals meeting criteria for developmental delays, mental health, and/or with intellectual disabilities.
Responsible for monitoring, linking, and evaluating services. Link to other services and make referrals as needed. Provide regular direct individual contact and provide face-to-face case management services. Meet productivity standards as set for the program. Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family. Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy. Obtain and update information within required timeframes. Provide crisis intervention services as needed. Work in cooperation with and consult with Agency staff, supervisors, other providers, and community resources as needed. Link individuals to transportation services or transport individuals to appointments as needed.
Attend meetings, staffings, and trainings as needed. At times, may provide coverage in Somerset County. Perform other related duties as assigned.
Work Schedule and Additional Information:
- Full-time employment.
- Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
- On occasion, staff may need to work hours outside of the set work week to complete projects, meet work demands or in the event of an emergency.
- Hours may vary to meet the needs of the consumers.
- This position reports to the DBHS Office in Somerset for work.
- Telework: Upon hire, new staff begin training and are on probation for 6 months. New staff may be eligible for a hybrid of on-site and remote work after 3 months of employment.

QUALIFICATIONS
Minimum Experience and Training Requirements:
- Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
- A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
- Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
- Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.
Other Requirements:
- PA residency requirement is currently waived for this title.
- You must be able to perform essential job functions.
