Minimum Requirements
Associate, Bachelor’s, Master’s or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Kentucky Physical Therapy license required to start the assignment.
BLS certification required from AHA or ARC.
Experience with OASIS documentation and HCHB/PointClickCare or similar electronic documentation systems required.
Home health care experience preferred.
Ability to perform start of care evaluations, re-certifications, return‑of‑care visits, and agency discharges within scope of practice.
Reliable transportation required. Mileage reimbursement provided between patient visits.
Assignment Details
Primary Specialty: Physical Therapist.
Facility Type: Home Health.
Shift: Days.
Shift Details: Monday–Friday, 0800–1600 (agency notes indicate schedule Monday–Friday 0800–1630 EST).
Weekend/On‑call: One weekend per month and one weekday on call per week as required by the agency.
Coverage Areas: Madison, Montgomery, Clark, and Powell counties.
Weekly hours: 40 hour guarantee.
