JR100441**POSITION SPECIFICS****Title:** Nurse Navigator**FTE:** 0.5FTE (20 hours/week)**Schedule:**Monday 8-4:30, Thursday 8-4:30, Friday 8-12**Holiday Rotation:** none**Weekend Rotation:** none**On Call Requirements:** none**POSITION SUMMARY**The Women’s Health Nurse Navigator serves as a specialized clinical guide for individuals undergoing breast or lung cancer screening, diagnosis, treatment, and survivorship care. This role provides personalized support by promoting timely access to care, assisting patients in understanding breast or lung health findings, and coordinating multidisciplinary services across the continuum. The navigator works closely with radiology, general surgery, oncology, primary care, and supportive services to ensure patients receive evidence‐based, compassionate, and coordinated breast health care.Additional responsibilities include supporting breast program development, quality improvement, community outreach, patient education, and collaboration with the SPH marketing team to promote breast and lung health awareness.**POSITION TECHNICAL RESPONSIBILITIES*** Serve as the primary point of contact for patients with abnormal imaging, symptoms, cancer diagnoses, or those requiring enhanced surveillance.* Guide patients through the care continuum, including screening, diagnostic imaging, biopsy, surgical consultation, oncology consultation, treatment, survivorship, and ongoing monitoring.* Collaborate closely with radiologists, general surgeons, oncologists, primary care providers, genetic counselors, and other specialists involved in breast care.* Provide education regarding imaging results, pathology findings, treatment options, and evidence‐based guidelines.* Coordinate multidisciplinary case review information and prepare concise patient summaries for care team discussion.* Facilitate appointments with imaging, surgery, oncology, genetics, lymphedema therapy, and supportive services within established service standards.* Assist patients in understanding their diagnosis, plan of care, and next steps recommendations,* Track key clinical milestones and reduce barriers to timely testing, diagnosis, and treatment initiation.* Document patient interactions and care plans using established navigation protocols and nationally recognized care guidelines.* Support data collection and reporting for quality metrics, accreditation standards (if applicable), and performance improvement projects.* Partner with marketing and public relations to develop community education programs, awareness campaigns, and outreach events focused on breast and lung health.* Provide education to staff regarding breast and lung navigation processes, emerging health technologies, and workflow updates.* Participate in task forces, committees, and interdisciplinary work groups related to program growth and quality.* Engage in continuous learning related to imaging, benign and malignant disease, risk reduction strategies, and survivorship care.* Perform other duties as assigned.**POSITION REQUIREMENTS****Education**:* Required: Associates degree in Nursing (ADN)* Preferred: Bachelor’s or Master’s degree in healthcare-related field highly desirable. Certification in Breast Health Navigation**Experience:*** Required: Three to five years of experience in applicable service line.* Preferred: Experience in patient navigation, oncology nursing, imaging services, or coordination of multidisciplinary care. Experience in community education or outreach.**Licenses and Registrations:*** Required: Registered Nurse (RN), State of Wisconsin* Preferred: CBE (Clinical Breast Examiner) competency.
