Job Summary and Qualifications
Performs direct patient care within the scope of the nurse practice act, as it applies to the outcomes and processes of the Heart Valve Center. Plans, coordinates and implements the care of the valve patient in the outpatient setting. Acts as a liaison with physicians, patients/families, and ancillary departments. Assesses financial, medical and social resources of current and potential valve program patients in conjunction with the Valve team. Coordinates the flow of information among the medical, nursing, financial and research staff as appropriate. Develops, maintains and implements a teaching plan with the patient and family, including lab values, medication overview and diagnosis‑related information.

Education: Graduate of an accredited School of Professional Nursing and current license by the Board of Nurse Examiners for the State of Texas.

Experience: Minimum 1 year of TAVR experience, preferred 3 years of Cardiology or critical care RN experience.

Compliance: Candidate completes initial and ongoing training and competencies as defined by service line, facility and department/unit specific to patient needs. Includes working knowledge of Microsoft Office, medical terminology and diagnostic imaging software. Requires RN license to practice in Texas or an active Multi‑State RN Compact License, or GN Permit for RN’s being hired into RN I position. BLS certification required.

TAVR experience required.

Essential Job Functions

Plan, coordinate, and implement care of valve patients in the outpatient clinic. Review clinical progress and coordinate communications with referring, valve and primary care physicians. Facilitate development of a patient‑specific teaching plan. Review current health information, relay information, and make referrals as necessary to the physician.

Serve as a resource for hospital staff regarding the care of valve patients. Educate patients and families about planned procedures, medications and treatments. Make referrals as appropriate per physician notes.

Develop and oversee implementation of internal policies and procedures in conjunction with standard policies. Interpret applicable laws, rules and regulations and ensure compliance.

Coordinate information among referring, primary care and valve clinic physicians, ancillary departments. Answer patient/family phone calls and field questions appropriately.

Participate in creating an environment that implements the mission, vision and values of the organization.

Perform full nursing assessment of clinic patients, including documentation of vital signs, signs and symptoms, NYHA class. Review diagnostic imaging studies, lab values and notify MD of abnormal findings.

Utilize and apply relevant computer programs, imaging and software.
