Virtual Work
This role enables associates to work virtually full‑time, with required in‑person training sessions, providing flexibility and autonomy. Alternate locations may be considered if candidates reside within commuting distance of an office.
Location and Shift
Locations: Kansas, Tennessee, Virginia, Iowa. Work Shift: Monday – Friday, 8:00 am – 5:00 pm CST with rotating on‑call.
Responsibilities
The Advance Practice Provider, Nurse Practitioner collaborates with physicians and providers to develop complex care plans and provides clinical support to patients.
Provides primary and urgent health care via telephone and tele‑video for patients receiving home and community‑based services.
Develops and implements clinical plans of care for adult patients with chronic or complex conditions.
Conducts history, physical exam, and diagnostics as needed and establishes treatment plans aligned with patient goals.
Identifies and closes gaps in care.
Works with interdisciplinary team to meet the patient’s and family’s physical and psychosocial needs.
Educates patients and families about medication usage, side effects, illness progression, diet, nutrition, adherence, and crisis prevention.
Maintains contact with other clinical team members, physicians, and providers to coordinate optimal care in timely manner.
Maintains patient medical records and documentation consistent with state regulations and company standards.
Participates in continuing education as required by state and certifying body.
Prescribes medication as permitted by state prescribing authority.
Minimum Requirements
Master’s degree (MS) in Nursing.
Active, national NP certification.
Valid, current, unrestricted Family or Adult NP license in Kansas, Tennessee, Virginia, or Iowa.
Valid, current, active RN Compact license.
At least 2 years of experience managing complex care cases.
Experience with Electronic Medical Records (EMR).
Preferred Skills and Experience
DEA registration or eligibility.
Active Medicaid number in Kansas, Tennessee, Virginia, or Iowa.
Experience managing complex care for developmental disabilities and chronically ill patients.
