Minimum Requirements
Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Indiana Physical Therapy license with no restrictions required to start the assignment.
Minimum of one year of Physical Therapy clinical experience required.
Basic Life Support (BLS) certification required.
Experience with home health documentation and workflows required, including HCHB and OASIS preferred.
Ability to perform start of care, resumption of care, regular visits, and recertification visits in the home health setting.
Must be able to travel within assigned coverage counties (Clark, Crawford, Floyd, Harrison, Jefferson, Scott, Washington).
Minimum education requirement: Bachelor's degree.
Preferred: HCHB experience and OASIS experience.

Shift Name: Days.
Shift Details: Monday–Friday, 8:00 AM–4:00 PM.
Weekly Hours: 40 hours guaranteed.
On‑call/Weekend Requirement: One weekend per month.
Mileage reimbursed between visits from first patient to last patient.
Primary responsibilities: HCHB and OASIS documentation, start of care, resumption of care, regular visits, and recertification visits.
