Position: Physical Therapist - Home Health
As a Physical Therapist in Home Health, you will complete assessments and evaluations of patient care needs related to functional status, mobility, endurance, pain and other medical conditions. Based on these assessments, you will work with the healthcare team to develop and implement a treatment plan, perform interventions aimed at improving and enhancing patient well‑being, and evaluate patient progress.
Primary Responsibilities
Evaluate the patient’s functional status and physical therapy needs; assist the physician in developing the therapy plan of care.
Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and coordinate with other healthcare team members.
Observe and report the patient’s response to treatment and any changes in condition to the supervising nurse and/or physician.
Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
Instruct and inform the health care team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Visit patients according to the plan of treatment, complete a progress note for each visit, and submit progress notes per company policy.
Complete all patient evaluations and develop the PT plan of care within state‑specific guidelines.
Report outcomes of evaluation, goals, and anticipated projected frequency of care.
Required Qualifications
Current Physical Therapy licensure in the state of practice.
Current driver’s license and vehicle insurance; access to a dependable vehicle or public transportation.
Commitment to providing care in any home setting regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Preferred Qualifications
Ability to manage multiple tasks simultaneously.
Ability to work independently.
Strong communication, writing, and organizational skills.
Pay and Benefits
Hourly pay ranges from $34.23 to $61.15 per hour for full‑time employment, depending on local labor markets, education, experience, certifications, and other factors.
