Overview
We are hiring an
RN Field Assessor
to conduct in-home PCA (Personal Care Assistant) assessments. This is a non-clinical, non-hands‑on role focused on evaluating patient needs, coordinating care, and ensuring appropriate service utilization.
Key Responsibilities
Conduct in‑home PCA assessments using standardized tools
Complete 5 assessments/day (20/week)
Evaluate medical necessity & appropriateness of care
Coordinate with patients, families, providers & care teams
Document assessments accurately in system
Participate in quality improvement initiatives
Prepare documentation for audits/hearings if required
Requirements
Active NJ RN license (Required)
2+ years clinical experience (Preferred)
Valid driver’s license + own vehicle (Required)
Comfortable with field/home visits (80% travel)
Basic computer skills (MS Office, documentation tools)
Field visits between 7 AM – 6 PM
Equipment provided (iPhone, tablet/laptop)
