Job Overview
In accordance with the objectives and goals of the Case Management Plan and State & Federal guidelines, the RN Case Manager Discharge Planner will be responsible for all aspects of case management for an assigned group of inpatients to determine the appropriateness of continued stay, assist in the development of the plan of care, ensure that the plan is implemented in a timely manner, and identify the expected length of stay (ELOS). The RN Case Manager works collaboratively with the inter‑professional team, including physicians, social workers, clinical nurses, home care services and others as needed.
Core Responsibilities
Conduct discharge planning and clinical review for an assigned group of inpatients using standardized criteria (MCG/InterQual) to achieve optimal outcomes and reimbursements.
Identify and document variance, avoidable days, and readmission reasons; document and escalates external/interqual barriers to discharge.
Assist in the development of the care plan and ensure timely implementation; discuss delays with the team and provide direction and support to move patients through continuum of care.
Collaborate with the Utilization Review team regarding medical necessity, patient status, and duration of hospitalization.
Work with medical
ursing personnel to influence appropriate progression of care and timely, safe discharge.
Participate in multidisciplinary patient progression rounds, focusing teams on progress toward discharge including needed tests, procedures, family education, and readiness.
Identify barriers to timely, safe discharge, develop plans to address them, monitor progress, and escalating if unresolved.
Coordinate services and equipment for post‑discharge care, ensuring patient choice of post‑acute vendors in accordance with guidelines.
Maintain accurate documentation of patient/family encounters in medical record and affiliated case‑management systems.
Provide information and resources to patients, families, and post‑hospital providers to maintain optimal health post‑discharge.
Stay current with concepts, research, and regulatory standards related to case management, utilization review, and discharge planning.
Qualifications
Minimum 2+ years of nursing experience (inpatient nursing, case management, utilization review, discharge planning, or community health nursing).
Associate’s Degree in Nursing (required).
Preferred:
Bachelor’s degree in Nursing.
Experience in utilization review and/or discharge planning.
Case Management certification.
Certifications and Licenses
Current licensure as a Registered Nurse in the State of New Jersey.
BSN preferred, but not required.
