Responsibilities
Under the supervision of the RN, performs delegated patient care activities as directed and ordered in accordance with training, competence and skill. Delegated tasks may include obtaining vital signs, height and weight, intake and output, calorie counts, and simple non-sterile dressing changes.
Safe patient handling: turning and positioning, use of specialty beds, patient transfer and use of transfer devices, ambulation assistance and use of assistive devices, restraint alternatives, oxygen use and transport, suction use, pre-operative skin preparation and/or use of sequential compression devices (SCD).
Specimen collection including blood collection for bedside glucose testing.
Maintain a safe patient environment: address hazards, hand hygiene, standard precautions, PPE per transmission-based precautions.
Communicate timely and accurate information about delegated patient care activities to the assigned RN, including observed patient responses, in accordance with hand-off guidelines.
Assist with data collection on admission and with transfer or discharge preparations as instructed.
Assist patients with activities of daily living, including bathing, shaving, oral care, toileting, grooming, changing linens, providing water, assisting with meals and feeding as directed.
Perform hourly rounding and use AIDET when interacting with patients; respond to call lights promptly.
Qualifications
Education: High school graduate or equivalent.
Experience: None required.
Training and competencies as defined by service line and department, per patient population served.
Preferred: Previous PCT, Nurse aide, or orderly experience; or any of the following on hire:
Completion of a CNA training program and CNA certification with the Texas Department of Aging and Disability Services.
Completion of an EMS program and EMT certification with the Texas Department of State Health Services.
Current LVN position in MHS as of Dec 1, 2011.
License/Certification: Basic Life Support (BLS) within 30 days of hire.
Essential Job Functions

As above: obtain vitals, intake/output, weight, and simple dressing changes; safe patient handling; specimen collection; maintain safe environment; communicate with RN; assist with admissions and transfers; assist with activities of daily living; hourly rounding and AIDET.
Additional Information
May assist with medication retrieval and delivery to licensed staff.
Support orientation of new personnel; uphold safety policies; report unsafe conditions; understand safety features of equipment; consult supervision for safe procedures.
Adhere to the MHS Code of Conduct and report wrongdoing as required by law and policy.
Other duties as assigned.
