Impactful LPN: Transitional Care & Care Coordination

Penn Highlands Healthcare is seeking a Licensed Practical Nurse to assist in call center functions while providing support in transitional care management and chronic disease management. The position requires coordinating patient care with physicians and outside healthcare providers. Qualified candidates will hold a current LPN license in Pennsylvania and may have experience with regulatory requirements and Microsoft applications. A sign-on bonus of $7,500 is available.
