
Nurse Practitioner

Key Responsibilities:
Conduct routine weekday rounding in assigned skilled nursing facilities
Perform focused patient assessments (“micro visits”) to evaluate recovery progress, pain, wounds, and functional status
Develop and implement individualized care plans in collaboration with physicians, therapists, and facility staff
Complete accurate and timely documentation using EHR systems (remote flexibility available)
Identify and manage readmission risks, ensuring continuity of care
Maintain compliance with post-acute care standards and PM&R-related regulations
Qualifications:
Master’s degree in Nursing (NP) or completion of an accredited Physician Assistant (PA) program
Active, unrestricted NP or PA license with prescriptive authority in the state of practice
Minimum 2–3 years of experience in geriatrics, post-acute care, PM&R, or related field (nursing facility experience preferred)
Strong clinical skills in managing recovery barriers, chronic conditions, and rehabilitation needs
Excellent communication and time-management skills
Proficiency with EHR systems and remote documentation
Ability to work independently with strong clinical judgment and consistency
