Centralia Palliative, part of LHC Group and the Optum family, is seeking a

Palliative Care Nurse Practitioner

to provide compassionate, specialty care to patients with serious illness who are not yet eligible for hospice. Join a mission‑driven team focused on quality, connection, and whole‑person care.

About the Role
You’ll provide palliative care consultations for patients with serious illness and a prognosis of one year or less, supporting patients and families through complex medical, emotional, and psychosocial needs.

What You’ll Do

Conduct comprehensive palliative assessments (physical, emotional, psychosocial, and spiritual)

Develop and manage individualized plans of care using evidence‑based palliative practices

Perform medication reconciliation and ongoing evaluation of care outcomes

Collaborate closely with patients, families, caregivers, and interdisciplinary teams

Deliver ethical, patient‑centered care across all stages of illness

Required Qualifications

RN/APRN license in Washington

Nurse Practitioner or CNS with prescriptive authority

BLS and current CPR certification

5+ years of clinical nursing experience

Valid driver’s license and reliable transportation

Preferred Qualifications

Advanced practice palliative certification (ACHPN)

1+ year of APRN experience in Palliative Care, Hospice, or Oncology
