Licensed Practical Nurse – Home Health Team
Job Summary
As a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll play a vital role in delivering compassionate, personalized care to patients in the comfort of their homes. In collaboration with the Registered Nurse and care team, you’ll help manage medical needs and promote patients’ independence – caring for patients like family, one home visit at a time.
Responsibilities
Providing nursing care in the home, including wound care, medication administration, and support during procedures
Documenting assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
Monitoring patient status and alerting the care team of any changes to ensure timely, coordinated responses
Educating patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home
Collaborating closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting
Qualifications
Basic Cardiac Life Support must be obtained within 15 days of employment start date
(DL) Driver License
(LPN/LVN) Licensed Practical or Vocational Nurse
Associate Degree, or Vocational School Graduate
1 year experience required
