Responsibilities
Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function
Perform therapeutic exercises and interventions to help clients regain independence and reduce pain
Assess and document patient progress, modifying treatment plans as needed
Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
Maintain accurate documentation and adhere to all legal, ethical, and professional standards
What We're Looking For
Licensed Physical Therapist – you must hold a valid license in the state of Michigan
Experience – while a minimum of one year in home health care is preferred, we’re open to fresh perspectives and new talent!
Team Player – you should have excellent communication skills and the ability to engage with patients and families effectively
Compassionate Caregiver – a genuine desire to help others and promote their independence is vital
Self‑Starter – the ability to work autonomously, manage your own schedule, and track patient progress is key
Transportation – a valid driver’s license and reliable vehicle for travel to patient locations
