Position Summary
Phoenix Heart is seeking a Licensed Practical Nurse (LPN) to support patient care within our cardiology practice. This role is responsible for assisting with clinical operations, patient assessment, care coordination, and supporting providers in the management of patients with cardiovascular conditions.

The LPN will play a key role in patient intake, symptom assessment, coordination of care, and ensuring timely communication between patients and providers to support quality outcomes and continuity of care.

Responsibilities

Provide direct patient care under the supervision of physicians and advanced practice providers

Perform patient intake including vital signs, medication reconciliation, and medical history updates

Assess patient symptoms via phone and in-office and communicate findings to providers
Prioritize patient needs and assist with scheduling urgent or same-day appointments as appropriate

Manage inbound and outbound patient calls related to symptoms, medications, and follow-up care

Support care coordination including follow-ups, referrals, and patient communication

Assist with medication administration and treatments within scope of practice

Monitor patient conditions and report changes or concerns to providers promptly

Provide patient education on medications, treatment plans, and when to seek additional care

Maintain accurate and timely documentation in the electronic health record (EHR)

Ensure compliance with clinic protocols, evidence-based practices, and regulatory requirements

Collaborate with providers, medical assistants, and administrative staff to ensure efficient workflow and patient care

Qualifications

Current Licensed Practical Nurse (LPN) license in the State of Arizona or compact state

Minimum of 1 year of clinical experience required; cardiology experience preferred

Strong clinical assessment and critical thinking skills within LPN scope

Ability to prioritize and manage multiple patient needs in a fast-paced environment

Knowledge of HIPAA, OSHA, and healthcare compliance standards

Proficiency in EHR systems and computer applications

Strong communication and interpersonal skills

Certifications

Current CPR/BLS certification required

ACLS preferred
