Overview

The Physical Therapist provides professional physical therapy services to home health clients in accordance with physician orders, the established plan of care, applicable state practice acts, licensure requirements, and organizational policies. This role performs comprehensive assessments, develops and implements individualized treatment plans, and delivers preventive, therapeutic, and rehabilitative interventions to promote functional independence and optimal outcomes in the home setting.

Responsibilities

Coordinate care with physicians and the interdisciplinary team; communicate changes in patient condition; ensure accurate and timely clinical documentation.
Provide patient and caregiver education, environmental assessments, discharge planning, and participation in quality improvement initiatives.
Supervise PTAs and Home Health Aides as permitted by state practice acts.
Demonstrate independent clinical judgment and adherence to regulatory and ethical standards; maintain a commitment to quality, compliance, and patient-centered care.
Minimum Qualifications

Graduate of an APTA-approved Physical Therapy program
Licensed Physical Therapist in the state of practice
Minimum one (1) year of experience or completion of a 6–8 week preceptorship
Home health experience preferred
Ability to work independently with minimal supervision
Commitment to maintaining clinical competence, organizational policies, and ethical standards
Valid driver’s license and reliable transportation required
Ability to meet physical demands of the role, including lifting up to 35 lbs and travel to patient homes
