Overview

Job Number: 26-6754 / 26-6792. Job Summary: Case Managers - Qualified Mental Health Professionals provide psychosocial rehabilitative skills training to individuals who have experienced chronic homelessness with at least one disabling condition, utilizing a recovery based and trauma-informed perspective at a site based Permanent Supportive Housing (PSH) at 3000 Oak Springs Drive, Austin, TX 78702. Case Managers operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill-set by forging therapeutic relationships with diverse community members.
Case Managers work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. They participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorder(s) at a regular interval to ensure optimal and personalized services. Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning.
Case Managers may provide all or some of the following: deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as medical care, nutritional assistance, financial assistance, transportation, school, legal assistance, and resources fulfilling any basic need. They may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidence-based interventions or therapeutic modalities. Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment.
This position may have additional duties associated with providing bilingual communication services and requires the completion of a language competency test. Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule. Responsibilities

Deliver therapeutic interventions fostering skills to manage symptoms, obtain/maintain employment or housing, and access services (medical care, nutritional assistance, financial assistance, transportation, school, legal assistance, and other basic needs).

Facilitate psychosocial rehabilitative skills training groups and utilize other evidence-based interventions or modalities.

Collaborate with an interdisciplinary team and coordinate services with external and internal providers.

Operate under supervision of a Licensed Practitioner of the Healing Arts (LPHA) who provides ongoing evaluation and oversight of assessments, diagnoses and treatment.

Support bilingual communication needs as required and complete language competency testing.

Maintain regular and predictable attendance and adapt to a flexible work schedule if needed.

Qualifications

Education: Graduated from an accredited college or university with a Bachelor's degree in Human/Behavioral/Social Services or related field; Master's degree preferred.

License: Current and valid driver's license and a clean driving record.

Experience: Preferred bilingual ability in Spanish.

Hours
Full-time. Workdays: Monday - Friday. Workhours: 9:00AM - 5:00PM, 8:00AM - 4:00PM.

Salary
$24.18 - $25.39 per hour, based on education.
