PAY RANGE
$63.00-$78.00 per hour

DESCRIPTION OF POSITION
The top priority of the Assessment and Referrals RN is to perform accurate assessment of incoming admissions in order to deliver high quality safe care during the Intake process. The RN comprehensively processes patients from receipt of referral until transfer to unit, assessing needs via crisis calls, walks or referral packets, and ensuring appropriate care per EMTALA guidelines. The RN uses facility policies, exclusionary criteria, and capabilities to accept patients for treatment, provides in‑person assessment, obtains MD orders, and determines appropriate unit assignment. The RN embraces SRBHH’s mission, vision and values, follows hospital policies and procedures, ensures compliance with regulations, and maintains knowledge and training. The RN also understands occupational health hazards such as bloodborne pathogen exposures.
KEY RESPONSIBILITIES
Respond to telephone and face‑to‑face inquiries from people seeking mental health care or information about services.
Use a systematic process of interaction with patients, their significant others, and health care providers to collect and analyze data regarding physical, psychosocial, environmental, self‑care, educational, spiritual, cultural, and anticipated discharge needs; determine drug toxicity, identify withdrawal signs, and conduct screenings or assessments to determine treatment needs.
Provide competent and timely care based on current chemical‑dependency and psychiatric nursing/clinical knowledge and standards; evaluate care against goals and revise as needed.
Facilitate the patient’s access to the appropriate level of care and setting.
Establish and maintain effective working relationships with patient services counselors, intake department, physicians, health professional affiliates, other facilities, and nursing staff.
Demonstrate skills in assessing crisis indicators via phone and in‑person and document calls on a call sheet, scheduling assessment as clinically appropriate.
Prepare written reports of assessment results promptly, including recommendations for level of care and treatment needs.
Make appropriate interventions during assessments and crisis situations with clients, family/friends and co‑workers.
Assess need for involuntary hospitalization, contact county crisis team, law enforcement or 5150 Certified employees as needed to ensure safety.
Enter data of calls and intake assessments in the computer system in a timely manner.
Assist clients not admitted to a program with follow‑through of clinical referrals and recommendations.
Provide pertinent clinical information to physicians and program staff to ensure a smooth transition.
Distribute documentation of admissions to the Business Office, Admitting Unit and Utilization Review Department.
Maintain knowledge of developmental tasks and sociological theories of pertinent age groups.
Maintain knowledge of legal guidelines pertaining to EMTALA/CBCOR and abuse
eglect reporting.
Communicate with managed care organizations to obtain insurance pre‑authorizations promptly.
Stay current with organization’s programs and services.
Participate in activities that enhance professional growth and development.
Perform other related duties as assigned.
Uphold the organization’s ethics and customer service standards.
Cross‑train as Inpatient Registered Nurse and perform all duties, including but not limited to: assess all inpatients’ status, provide direct care to assigned group of inpatients, document care, administer medications as needed, and ensure necessary documentation and treatment plans are completed at shift’s end.
Provide clinical summary information at meetings, shift reports, and hand‑offs; oversee inpatient psychiatric unit operations.
Display ability to assess, collect data, define problems, set goals, initiate interventions, and evaluate interventions.
Provide patient care by demonstrating knowledge of mental health diagnoses, standard of care, evidence‑based practice, high risks, National Patient Safety Goals and care models.
Knowledge and Experience
Current Registered Nurse in the state of California.
One year of nursing experience or more required.
Inpatient experience preferred.
Familiarity with continuum of care and clinical terminology.
Knowledge of DSM diagnostic criteria.
Current CPR certificate or obtained during initial orientation.
Verbal de‑escalation and physical intervention program as approved by the CNO.

Physical Requirements
While performing the duties of this job, the position frequently requires the following:
Use standard office equipment and access, input, and retrieve information from a computer; use keyboard with manual and finger dexterity and wrist‑finger speed sufficient for repetitive tasks over extended periods.
Communicate effectively in person or via telephone, ensuring clarity for a diverse population.
Give and follow verbal and written instructions with attention to detail and accuracy.
Perform complex mental functions and basic arithmetic functions; interpret laws, regulations, and policies; collect, interpret, and analyze complex data.

Minimum visual acuity standard, with or without correction.
