Summary
The Registered Nurse (RN) provides comprehensive skilled care as ordered by the attending physician. The RN educates patients, their family members, and ensures the safety of the patient. The RN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The RN is responsible for all practices and duties within the scope of practice as outlined by the state.

Qualifications & Attributes
Registered Nurse with current license in the state of employment.
Minimum of one year of experience as an RN in an acute care setting; home‑care experience is desired.
Valid driver’s license and auto insurance listed in your name.
Effective communication skills and the ability to create positive interactions with patients, families, physicians, and co‑workers.
A calm, patient, accommodating, compassionate, and caring disposition.
Knowledgeable in nursing best practices and able to make appropriate nursing judgments.
Detail‑oriented, observant, disciplined work ethic with the ability to prioritize and be timely.
Ability to follow directions and work collaboratively as a team member.
Capability to meet all physical demands and satisfy health qualification requirements.
Major Areas of Responsibility
Provide skilled nursing care in patients’ homes as ordered by the attending physician.
Observe and monitor patient conditions; perform OASIS assessments to develop individualized care plans and adjust as needed.
Administer medications as prescribed by the physician.
Help reduce rehospitalizations by front‑loading visits for high‑risk patients and teaching preventive measures.
Manage multidisciplinary care; promote continuity of care with admissions, transfers, and discharges.
Counsel patients and families on disease/injury processes and self‑management.
Incorporate patients and families in the development of care plans.
Supervise total care provided by aides and LPNs; communicate supervisory visit results.
Complete all clinical documentation in line with agency protocol and Medicare/Federal guidelines.
Maintain an active RN license in the state of employment.
Communicate with schedulers regarding any changes outside the normal schedule.
Participate in case conferences, on‑call requirements, and agency meetings; attend in‑service training.
Maintain a professional appearance and adhere to agency policies and procedures.
Essential Functions
Drive to patient’s primary location per scheduled visit.
Document all aspects of care within required timeframes (e.g., discharge, evaluation, recertification, or resumption of care visits within 24 hours).
Follow the plan of care within the scope of practice for an RN and fulfill visits in the assigned geographic area.
Health Qualifications
Must be able to meet all physical position requirements.
Must have a current negative tuberculin skin test or equivalent, with annual testing thereafter.
Must be free of physical or medical conditions that would limit or restrict job functions.
May be exposed to bloodborne pathogens.
Travel is required.
