Description
Perform a comprehensive nursing assessment upon admission of the patient including physiological, psychological and environmental factors as well as health history to provide for the safety and quality of care. Integrate all pertinent data gathered on assessment and identify problems that form the basis of a plan of care, adhering to case management objectives.

Maintain annual HHRG values at the National Standard as appropriate.
Utilize HHRG values to establish visit frequency and to establish appropriate interdisciplinary utilization.
Annual outcomes meet National Benchmarks.
Refer to community resources as appropriate.
Evaluate the clinical needs of the patient; initiate, develop, implement and make necessary revisions to the patient's plan of care; address all problems in the plan of care and document accordingly.
Participate in interdisciplinary patient case conferences; evaluate the effectiveness of the established plan of care.
Instruct and provide documented supervision of the Home Health Aide plan of care every 14 days.
Notify MD/other involved care providers of significant changes in the patient's condition.
Complete admission and other paperwork on a timely basis.
Record narrative documentation to reflect skilled care. Documentation is clear, concise and addresses specifics of teaching, direct care and response of patient/caregiver.
Obtain and clarify valid MD orders for all care delivered.
Plan and coordinate discharges appropriately and inform patient/MD and record disposition of patient in the discharge summary. Complete all appropriate discharge paperwork.
All other duties as assigned.
Qualifications
Successful passage of VNA pre-hire medication test.
Current RN license in Mass. and other related credentials to meet specialty.
Minimum one (1) year of nursing experience required or 6 months of nursing experience with a BSN.
CPR certification required.
Home care and IV experience, computer skills and BSN preferred.
Schedule Details
22.5 Hours Per Week, Monday–Friday, 8a–4p or 9a–5p.
Occasional Weekends and Occasional Holidays.
Occasional Overnight/Weekend On-Call Responsibility.
Area: Nantucket.
Certified Home Health and Hospice.
Organization: Visiting Nurse Association.
Primary Location: Massachusetts-Nantucket.
Department: VNA-RN Hourly Union.
Annual/Hourly: Hourly.
Hiring Pay Range: 33.7 - 55.01.
