Working closely with a dedicated team of physicians and clinicians, you’ll develop and manage care plans that support recovery and help patients get back to the life they love.
Responsibilities
As a
Home Health LPN , provide professional nursing services under the direction of an RN in compliance with the state's Nurse Practice Act, licensure/certification requirements, applicable healthcare standards, governmental laws and CenterWell Home Health policies.
Provide skilled nursing interventions for the patient/client illness, rehabilitative needs and preventative care; utilize a holistic approach in implementing the plan of care, teaching and training as appropriate.
Apply knowledge and skills to facilitate problem resolution and achieve individualized patient goals and outcomes.
Confer with the RN Supervisor on changes to the Plan of Treatment; accept verbal orders from physicians where permitted by state law and communicate these orders to the RN Supervisor.
Implement the nursing plan of care using a systematic, individualized, outcome-driven approach.
Maintain timely communication with patients, physicians, clinical managers, and other team members regarding patient/family needs and status changes; participate in care coordination and discharge planning as appropriate.
Maintain professional conduct and confidentiality; share information only when appropriate and in the patient’s best interest.
Ensure availability of supplies, materials and equipment and communicate to support adherence to professional standards in care delivery.
Prepare and submit documentation as required by the Company; visit/shift notes documented on day services rendered.
Required Experience/Skills
Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Current nursing license in the practicing state.
Valid driver’s license, auto insurance and reliable transportation.
Current CPR certification.
Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
Pay Range
• $33.00 - $47.00 pay per visit/unit
• $53,000 - $73,000 per year base pay
