
Licensed Behavioral Therapist (Consultant)- LISW, LPCC

Job Summary
This role is a core member of the integrated care team. The role provides short-term, individualized clinical interventions to members who have been identified as seeking therapy and having a behavioral health condition (e.g., substance use, depression, anxiety, schizophrenia, bipolar) that negatively impacts their health and daily functioning. Centered in a trauma‑informed care approach with emphasis on harm reduction and motivational interviewing, the role elicits behavioral change and adherence with medical recommendations and treatment plans.
The BHC serves as the behavioral‑health subject matter expert within an interdisciplinary care team that includes primary care providers, nurses, medical/social work care managers, psychiatrists, pharmacy, community health workers, and resource specialists. The BHC may refer members to behavioral‑health levels of care outside of AbsoluteCare (e.g., ACT, IOP, PHP, residential treatment, inpatient services). The BHC assesses members through a biopsychosocial approach and develops a person‑centered treatment plan with measurable goals, improving physical and behavioral health. Services include short‑term individual therapy, group therapy, and post‑hospital follow‑up within 7 days of discharge (HEDIS measures).
Key performance indicators include meeting treatment‑plan goals, increased adherence to treatment, reduced preventable medical/psychiatric events (e.g., ER visits, hospital admissions/readmissions), post‑hospital follow‑up measures (HEDIS), and member retention. The BHC promotes AbsoluteCare’s mission, vision, and values in all interactions.
Duties And Responsibilities
Provide individual, group, and post‑discharge follow‑up therapeutic services that include comprehensive biopsychosocial assessments, individualized SMART‑goal treatment plans, timely clinical documentation (e.g., SOAP notes), and tracking of progress to completion.
Collaborate with staff or provide crisis intervention and de‑escalation for members in crisis (telephone or in person), including suicidality and disruptive or threatening behaviors, and determine disposition as clinically appropriate.
Serve as the clinical behavioral‑health SME within the interdisciplinary team by making recommendations, referrals, and follow‑up for behavioral‑health aspects of member care.
Advocate for and deliver evidence‑based approaches, including trauma‑informed care, harm‑reduction, motivational interviewing, and cognitive‑behavioral therapies. May provide short‑term solutions‑based therapy or emotional management such as anger management.
Model trauma‑informed care and motivational interviewing in member interactions and staff trainings.
Maintain communication with PCP and psychiatrist for all members seen for therapy.
Complete documentation of patient care services in the electronic health record within 3 business days of interaction.
Develop or support treatment plans for members with chronic mental illness (schizophrenia, bipolar, depression, anxiety).
Recognize opportunities to promote long‑acting injectables when appropriate to support adherence to treatment goals.
Schedule appointments and promote attendance, which may include reminder calls, address barriers such as transportation, and follow‑up after missed appointments.
Collaborate with clinical leadership to develop and maintain group‑therapy curriculum, covering educational and process components such as grief, anger, anxiety, aftercare, overall health/well‑ness, and tobacco cessation.
Minimum Qualifications
Clinical license in the state of practice with at least 3 years of experience in a behavioral‑health field (social work, mental health, or addiction).
Licenses that allow clinical practice (e.g., LISW, LPCC).
At least 2 years of experience in a community mental health center, FQHC, inpatient psychiatric facility, or similar setting serving complex behavioral‑health needs.
Experience with complex government‑sponsored populations (Medicaid, Medicare) preferred.
Experience with member engagement, transitions of care, interdisciplinary clinical care, or case management preferred.
Telehealth experience preferred.
Excellent clinical judgment and ability to manage an independent caseload.
Advanced computer skills, including Outlook, Word, PowerPoint, Excel.
Excellent written and oral communication skills and the ability to adapt language to a member’s comprehension level.
Experience using multiple electronic health records.
Excellent organizational and time‑management skills for triage and prioritization.
Proven ability to work in a fast‑paced, dynamic environment.
Active CPR certification.
Working Conditions
This job operates in a professional office environment and routinely uses general office equipment.
Physical Requirements
Communicate clearly and exchange accurate information continuously.
Remain stationary for long periods of time.
Operate computer, keyboard, copier, fax machine, phone, and other general office equipment constantly.
Occasionally move objects up to 20 lbs.
