As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.
You will work onsite at our New Conviva Port Salerno Center in Stuart, Florida, and report to our Associate Medical Director.

Your Role & Responsibilities
Provide comprehensive care to seniors following clinical standards.
Collaborate daily with your care team to ensure coordinated, high-quality care.
Manage referrals, hospital/SNF coordination, DME, and home health services.
Document care accurately and efficiently with support from quality-based coders.
Participate in on-call rotation and support growth plans at your center.
Have a passion for providing value-based care.
Required
Master's Degree in Nursing or completion of a PA program with board certification.
Active, unrestricted NP or PA license in the state of practice.
Screened for TB if selected for this role.
Preferred
Experience in value-based care, primary care or Medicare Advantage settings.
Active DEA license and Medicare/Medicaid provider numbers or ability to obtain them.
Familiarity with HEDIS quality indicators and Medicare guidelines.
Location: Conviva Port Salerno, Stuart, Florida.

Language proficiency testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.
Scheduled Weekly Hours: 40.
Pay Range: $123,800 - $170,300 per year. This job is eligible for a bonus incentive plan based on company and/or individual performance.
