Overview
Wound nurse needed for Woodstock Nursing and Rehabilitation. This is a full-time wound care RN/LPN position. The wound care nurse participates in assessing and evaluating the clinical needs of residents with wounds, and works with the resident and the care team in planning and implementing care that addresses the needs. This work supports the mission of Woodstock Nursing and Rehabilitation Center.
Hours
Full-time Monday–Friday (8:00 a.m.–4:00 p.m.). Includes every other weekend.
Responsibilities
Conduct comprehensive wound consultations including detailed wound assessments and recommendations for interventions and care planning.
Conduct wound assessments for residents and complete all associated documentation, tracking, and care planning activities.
Ongoing monitoring of all assigned residents with wounds.
Assess resident/caregiver knowledge of wound prevention and treatment. Provide resident/caregiver education on wound prevention and treatment as needed.
