Job Summary
Provides professional nursing care to patients across the age spectrum within an assigned unit. Supervises and coordinates care planning with other disciplines. This position is represented by RCHEA.

Responsibilities
Assesses patient needs, problems and concerns. Performs data collection in an ongoing systematic manner, focusing on physiologic, psychosocial, cultural, spiritual and cognitive status, and including nutrition, pain, patient/family education, family involvement and patient advocacy. Assesses patient, family and caregiver learning needs and readiness for learning.
Evaluates effectiveness and outcomes of care given, and adjusts plan of care process and provision as necessary with consultation as needed. Demonstrates succinct, legibly written documentation skills on all medical records and forms. Consistently documents pertinent information based on changes in patient condition/response and treatment provided. Navigates electronic medical record system to document completely and precisely. Participates in nursing research activities as requested.
Formulates a goal‑directed plan of care that is prioritized and based on patient diagnosis and anticipated patient outcomes. Individualizes updates and documents the plan of care. Includes the patient/family/designee (e.g., POA) in the planning process. Establishes priorities based on essential patient and family needs while incorporating available unit resources. Educates patients, families and caregivers appropriately on plan of care.
Implements care in a thorough, safe, skillful, consistent and continuous manner. Delegates appropriately and coordinates duties of health care team members. Regularly helps with orientation and training of new and existing employees and students, assisting them in learning nursing unit routines, policies and procedures, etc. Ensures compliance of team with applicable laws, rules and regulations. Communicates effectively with patients, families and members of the health care team while upholding a positive professional manner and service philosophy.
Maintains and meets expectations on time for all competencies, license, certifications and education requirements as outlined by local administration, Adventist Health (AH), The Joint Commission (TJC), Centers for Medicare and Medicaid Services (CMS) and all other regulatory agencies.
Willingly performs other duties and innovations as assigned.
Qualifications
Graduate from an accredited school of nursing.
One year of experience in a related field required for RN II‑IV.
Current California RN license.
Basic computer skills.
Current knowledge and skills appropriate to the age/type of patient population.
Must have BLS certification; recertify every two years.
Must have ACLS certification; recertify every two years. Must obtain within six months of employment for specific units.
Must have PALS certification (specific units); recertify every two years. Must obtain within six months of employment for specific units.
All BLS, ACLS, PALS and NRP certified personnel must familiarize themselves with the crash cart.
Physical Requirements
Requries frequent bending, squatting, kneeling, climbing, and work with arms above shoulder height. Requires frequent transferring of patients and demonstration of a pull lift of 50 pounds from the floor.
Vision requirements include close vision and the ability to adjust focus.
Must be able to drive or climb ladders? (Not required for this position.)
