Centene is hiring an LTSS Service Care Coordinator for our Illinois market, supporting our Medicaid Traumatic Brain Injury and HIV/Aids population. In this role, you will conduct in-home and some facility visits to complete assessments, coordinate care, and assist members with connecting to providers and services.

This is a 75% field-based position covering the following areas in Illinois: Elgin, Hoffman Estates, Lincolnwood, Morton Grove, Niles, Schaumburg, Skokie, Streamwood.

Schedule: Monday through Friday, 8:00 a.m. – 4:30 p.m.

Position Purpose

Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost‑effective healthcare outcomes.

Makes personalized service care plans for long‑term care members and educates members and their families/caregivers on services and benefits available to meet member needs.

Evaluates the member’s needs, the resources available, and recommends/facilitates a plan for the best outcome.

Assists with developing ongoing long‑term care plans and identifies providers, specialists, and community resources needed.

Coordinates between the member/family/caregivers and the care provider team to ensure services are accessible.

Provides resource support for members and families on needs such as employment, housing, participant direction, independent living, justice, foster care based on assessment and plans.

Monitors care plans, member status and outcomes; recommends adjustments as needed.

Interacts with long‑term care healthcare providers and partners to meet member needs.

Collects, documents, and maintains member information and care management activities to ensure compliance with current state, federal, and third‑party payer regulations.

Might perform on‑site visits to assess member needs and collaborate with providers or resources.

Provides or facilitates education to members and families on procedures, provider instructions, service options, referrals, and benefits.

Provides feedback to leadership on opportunities to improve quality of care and service delivery cost‑effectively.

Performs other duties as assigned.

Complies with all policies and standards.

Education / Experience
Requires a Bachelor’s degree and 1 year of related experience, or equivalent gained through accomplishments reflective of the level of this position.

For the Illinois Plan: In addition to the above requirements, the employee will work on the Brain Injury/HIV/AIDS population. Candidate must meet one of the three following criteria:

Registered Nurse (RN) licensed in Illinois with a bachelor’s degree in nursing, social work, social sciences or counseling, OR four (4) years of case management experience.

Certified or licensed social worker with a bachelor’s degree in social work, social sciences or counseling, or a Master’s in social work.

Unlicensed social worker with a bachelor’s degree in social work, social sciences, or counseling.

Additionally, must have experience working with:

Addictive and dysfunctional family systems

Racial and ethnic minorities

Homosexuals and bisexuals

Persons with AIDS

Substance abusers

Pay Range: $22.94 - $38.79 per hour.
