Purpose of Job

Improve the overall health of the communities we serve by providing social or behavioral health services to patients and families.
Compensation Range

$73,200.00 - $110,722.00
Benefits

Annual milestone bonus: $500 for each year worked, up to $5,000.

Essential Duties and Responsibilities

Provide individual and group counseling within scope of practice to aid clients in personal, social, educational, and vocational development.
Available to primary care providers for behavioral health staffing and consultation.
Collect psycho‑social data and provide information to providers to assist in medical care management.
Perform mental health evaluations using approved tools to assess amenability to treatment and required level of care.
Develop detailed care plans and document them in the electronic health record.
Link clients with community resources.
Provide in‑service training to CHAS staff as requested.
Participate in community outreach activities as directed.
For Staff With Chemical Dependency Expertise

Perform chemical dependency assessments using approved instruments and determine referral suitability.
Conduct individual and group counseling focused on alcohol and drug treatment, relapse prevention, and cognitive behavioral change.
For Staff With Clinical Supervision Training and Duties

Supervise and coordinate volunteers and behavioral health/social work students as required.
Facilitate peer review and chart audit processes as assigned.
Perform other duties assigned, supporting the CHAS Health mission and core values.
Qualifications

Education/Experience: Valid state licensure as a Licensed Advanced Social Worker (or associate), Licensed Independent Clinical Social Worker (or associate), Licensed Mental Health Counselor (or associate), Licensed Marriage and Family Therapist (or associate), Licensed Clinical Professional Counselor, and/or Licensed Professional Counselor. Minimum two years of clinical practice with underserved populations (desired).
Skills: Knowledge of community linkage resources; excellent oral and written communication; computer proficiency; BLS (CPR/AED).
Additional: Valid driver’s license and insurance; commitment to a safe and respectful environment.
Physical Demands

Must talk, hear, and sit more than two‑thirds of the day. Use hands over two‑thirds of the day; reach with arms less than one‑third of the day. Communicate over two‑thirds of the day; minimal climbing, stooping, or crawling. Lift or move up to 10 pounds regularly, and occasionally up to 25 pounds. Reasonable accommodations available.
