Responsibilities
Evaluate assigned patients/residents and develop treatment plans with specific goals and activities.
Reevaluate and modify treatment plans as necessary.
Ensure treatments and care are done appropriately and completely in accordance with the patient’s clinical condition.
Provide instructions in therapeutic procedures to be continued by the patient as prescribed by a physician.
Provide direct physical therapy to patients/residents.
Communicate clearly and tactfully with patients/residents regarding progress, problems, home programs, and other issues related to the therapeutic process.
Communicate observations, facts, and comments with interdisciplinary team members individually and through team meetings as appropriate and required.
Assess patients’ progress weekly or as required and adjust programs accordingly.
Prepare written documentation as required by regulations and departmental policy, such as evaluations, individualized treatment plans, progress notes, and other required reports.
Maintain established departmental policies, procedures, objectives, and quality assurance.
Perform duties for patients aged adolescent through older adult.
Qualifications
Current California Physical Therapy License
Current BLS CPR certification
May be exposed to infectious and contagious diseases, body fluids, and wastes.
Heavy physical effort, including lift/carry up to 50 lbs.
Prolonged standing, walking, bending, and squatting.
Must be able to lift, position, and transfer patients/residents and equipment.
Handles emergency or crisis situations as needed.
Salary
$45.08 - $69.86 hourly. Individual wages are determined based upon the individual’s qualifications and experience.
