Licensed Practical / Vocational Nurse – LPN/LVN
Our Local Office is looking for a compassionate LPN/LVN to provide skilled nursing care for a special client. Available shift: Saturday 6:30 pm–12:30 am. Location: San Antonio, TX 78221.
Position Overview
The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse, in accordance with the patient care plan and physicians’ orders, while maintaining confidentiality and professional boundaries.
Essential Job Functions
Deliver and coordinate quality patient care in compliance with physician orders.
Observe, assess, and report changes in patient condition to the supervisor and/or physician.
Document all activities, assessments, nursing actions, responses, and care coordination in a timely manner.
Participate in implementing and updating the nursing care plan.
Take appropriate nursing action based on assessment and achieve expected outcomes.
Recognize changes in patient needs and respond with appropriate intervention.
Maintain compliance with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
Provide care using infection control measures that meet OSHA standards.
Educate patients and families about disease processes, self‑care techniques, and prevention strategies.
Keep knowledge of nursing competencies by participating in educational programs and continuing education.
Requirements
Graduate of an accredited school of nursing.
Current, unrestricted state license as an LPN/LVN.
Current CPR certification.
Proficient in clinical assessments, documentation, and compliance with nursing care policies and procedures.
Additional State Specific Requirements
South Carolina – one (1) year of pediatric experience.
California – one (1) year of experience working under the current nursing license.
Louisiana – one (1) year of experience working as a licensed nurse.
Continuing education as required by state.
Preferences
Six (6) months of recent experience as an LPN/LVN in a clinical care setting.
Home health experience.
Other Skills/Abilities
Attention to detail.
Time management.
Effective problem‑solving and conflict resolution.
Good organization and communication skills.
Physical Requirements
Must be able to speak, write, read, and understand English.
Must be able to travel.
Must be able to lift 50 pounds.
Must be able to reposition patients and move equipment without assistance.
Must tolerate prolonged walking, standing, bending, kneeling, reaching, twisting, and climbing stairs.
Must have visual and hearing acuity.
Must be able to respond physically and mentally to emergency situations in the home or during transport.
Environment
Must function in a variety of environments, including exposure to allergens and other conditions.
Possible exposure to blood, bodily fluids, and infectious diseases.
