Overview
The office and patient coordinator provides quality administrative and clerical services for program staff and assists Senior Life Solutions patients with care needs. They support patients’ general wellbeing, physical care and daily transport while ensuring compliance with program regulations. The role enhances the patient experience and contributes to a positive team environment.
Responsibilities
Assist with insurance verification and billing procedures, including monthly verification forms.
Provide clerical support to program staff and coordinate documentation such as daily nursing checklists, multidisciplinary progress notes, medical records filing, and form tracking.
Assist patients with the completion of consents, admission processes, vital sign measurement, sit‑to‑stand tests, census reporting, and everyday care tasks.
Provide safe transportation to patients in a company vehicle as required, ensuring vehicle is clean, temperature‑controlled and logs remain accurate.
Maintain inventory of supplies, cleanliness, group‑room setup, and refrigerator temperature logs.
Participate in treatment team meetings and coordination of care, including meal and snack planning.
Support community engagement initiatives, ongoing community education contacts, and customer‑service training related to community integration.
Contribute to a cohesive team environment to ensure program success, following 25‑point compliance checklist and other program regulations.
Qualifications
High School Diploma or GED required.
Preferred CNA, MA or LPN certification.
Experience providing care to older adults in a hospital or nursing facility setting.
Experience performing secretarial or clerical duties; proficiency with computers and electronic medical records.
Good driving record, comfort driving a van, and ability to transport patients.
Knowledge of community resources for older adults, familiarity with medical terminology and medical team roles.
Working Conditions
Office and Patient Coordinator will be exposed to viruses, disease, and infection stemming from patient interactions.
Physical Requirements
Regular talking, hearing, & visual acuity including close, distance, color, peripheral, and depth vision.
Intermittent physical activity: standing, walking, bending, kneeling, stooping, crouching, lifting, driving, and patient support.
