Description
This is a flex nursing role which provides case management, home health and clinic visits in Napa county.
Schedule
Weekdays Monday to Friday 8am-4:30pm. No weekends or holidays!
Responsibilities
As a member of the Interdisciplinary Team, the Registered Nurse (RN) participates in the development of an integrated plan of care and provides direct nursing care to frail elderly participants. The Flex RN position supports and provides coverage for the PACE RN Case Manager, PACE clinic nurse, and home health nurse positions. This dynamic position utilizes the full spectrum of high level nursing skills – from critical‑thinking care coordination and triage to hands‑on, clinical nursing skills.
Required Qualifications
Education sufficient for licensure
Within 30 days of hire: National Provider BLS – American Heart Association
Upon hire: California Registered Nurse License
1 year long‑term care or geriatric nursing
1 year working with the frail and elderly
Preferred Qualifications
