Responsibilities

Evaluate patients to determine rehabilitation needs, potential for achievement, and develop a plan of care.

Implement and develop the plan of care to ensure quality and continuity of care and proper discharge planning. Conduct initial evaluations and re‑evaluations at each visit. Perform follow‑up assessments per OASIS and PPS regulatory guidelines.

Communicate significant findings, problems, or changes to supervisors, physicians, and other team members involved in patient care.

Initiate patient safety plans using patient, family, and community resources.

Verify the plan of care prior to each visit and deliver care according to physician orders, assessment data, and established guidelines.

Write physician orders to cover additional visits or changes to the plan of care, per agency policy.

Integrate patient‑care goals into therapy sessions and document in therapy notes.

Provide occupational therapy services according to accepted standards of practice and with physician certification.

Teach patients or families activities of daily living, using adaptive equipment, sensory integration, and fine‑motor coordination skills.

Assist patients or caregivers in securing necessary equipment, including fabrication of custom splints.

Provide patients and/or caregivers with a home program, including modification and clarification when necessary.

Counsel patients and families to meet therapy and related needs.

Supervise or provide clinical direction to Certified Occupational Therapy Assistants, Home Health Aides, and clinical staff to ensure the therapy care plan is followed and quality of care is maintained.

Work in a physically demanding, high‑stress environment requiring full range of motion, lifting, pushing, pulling, and transferring patients, and handling equipment.

Possibly work irregular hours, including call hours if applicable.

You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.

Requirements

Graduate of an accredited Occupational Therapy program.

Current, unrestricted OT license valid for the state of work.

Minimum one year of experience in a clinical care setting.

Able and willing to travel within branch coverage area, with a dependable vehicle, valid driver’s license, and current auto insurance.

Will travel up to 50% of the time.

Ability to lift, push, pull, and support up to 50 pounds frequently, including positioning or transferring patients and moving equipment.
