Concho Hearts Hospice is looking for a compassionate Social Worker (LMSW or LBSW) to provide PRN hospice social services in the Corpus Christi, TX area.

What You’ll Do
Provide emotional support and counseling to hospice patients and families.
Assess psychosocial needs related to terminal illness and collaborate with the RN Case Manager and interdisciplinary team.
Develop and guide individualized plans of care.
Connect families with community and bereavement resources.
Help patients and caregivers navigate life’s most meaningful journey with compassion and dignity.
What You’ll Need
LMSW or LBSW license in Texas (in good standing).
Master’s or Bachelor’s degree in Social Work from an accredited program.
At least 1 year of experience in long-term care or acute/medical-surgical setting (hospice experience preferred).
Strong communication, organization, and EMR documentation skills.
CPR certification and reliable transportation.
