Case Manager – Washington County – Pinnacle Services
A dynamic and collaborative role in a rapidly growing agency, focusing on person‑centered care for individuals with developmental and mental health needs across six metro counties.
Job Responsibilities
Assess life areas where assistance is needed and develop individual service plans.
Co‑ordinate and offer resources to support persons served.
Monitor service delivery, ensuring providers meet client needs.
Attend regular interdisciplinary meetings with clients.
Document client interactions to create billable units.
Salary
Non‑LSW wage: $49,000 / year (salaried, 40 hrs/week). LSW wage: $51,600 / year (salaried, 40 hrs/week).

Qualifications
Graduate of an accredited four‑year college with a major in social work, psychology, sociology, or a closely related field – or any field with at least one year of full‑time experience as a social worker/case manager/care coordinator.
One year of experience in education or treatment of persons served with developmental disabilities or related conditions.
At least one course focused on developmental disabilities (transcript required).
Valid driver’s license, acceptable driving record, and insured vehicle.
Successful completion of local and/or state background checks.
Preferred Qualifications
Possession of LSW or higher BOSW licensure.
