Job Description Summary
An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or in an alternate infusion suite, ensuring patient safety with continuity and compliance under a physician’s plan of care. The role works under structured supervision of the designated supervisor.

Job Responsibilities

Initiate, develop, and implement intermediate nursing plan of care treatments, evaluate patient progress towards goals, organize and participate in direct patient care, perform treatments, administer medications, and educate patients and families/caregivers.

Assess patient needs and physical status at each skilled visit, re-evaluate needs through reassessment, respond to therapy, and incorporate supplemental physician orders; obtain and clarify physician orders for treatment revisions, inform physicians promptly of significant changes, and provide written summaries.

Provide training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance.

Deliver effective patient‑centered education to help patients and families achieve independence with prescribed therapy and care needs.

Communicate effectively and timely with Option Care’s clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care; prepare real‑time clinical documentation, submit to the nursing department in compliance with billing timelines, and ensure legible writing using approved abbreviations.

Coordinate discharge planning, prepare discharge summaries with patient instructions, and report patient care needs, progress, and goals when transferring care.

Demonstrate compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations, and accreditation standards.

Maximize work efficiency using computers and automation technologies to validate treatment orders, communicate patient care, and adhere to the assigned visit schedule.

Respond promptly to patient requests; initiate emergency procedures as necessary.

Accept accountability for own practice through ethical and professional conduct; follow established programs and procedures in line with Option Care’s mission, values, and objectives.

Observe legal and ethical guidelines safeguarding confidentiality of patient and proprietary information, including HIPAA compliance.

Explain Option Care’s scope of services and related financial obligations for care and service charges.

Attend required training modules, in‑services, and continuing education to maintain competency and licensure; oversee paraprofessionals in the home setting and may act as Supervisor in the absence of the Supervisor or Nurse Manager, consistent with state and federal regulations.

Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses for orientation and supervisory activities as assigned.

Assist with nursing department on‑call responsibilities.

Perform other related duties as directed by supervisor.

If Applicable

Insert PICC/Midline per physician order.

Troubleshoot or declothe obstructed central line catheters.

Monitor PICC/Midline and educate on prevention of line infection and routine care.

Supervisory Responsibilities

Basic Education and/or Experience

Active and unrestricted RN license in the state of practice.

Minimum of 1 year of nursing experience.

Current CPR certification required.

Basic Qualifications

Demonstrated competency in patient care standards for safe delivery of services and infusion skill sets applicable to agency programs and service needs; advanced certification and training as applicable.

Required licensure to operate a motor vehicle in the state of practice with access to a vehicle for business travel and proof of liability insurance.

Basic knowledge of computer operating systems and software applications, and ability to apply knowledge in effective use of nursing technology tools to communicate and document care provided.

Physical Demand Requirements

Ability to lift up to 50 pounds, with a maximum lifting of 75 pounds; exert up to 20 pounds of force occasionally or 10 pounds frequently.

Ability to walk, stand, crouch, kneel, turn, pivot, balance, stoop, reach overhead, grasp, push, pull, lift, and carry; fine motor skills and visual acuity for close and distant work, color and peripheral vision, depth perception, and focus adjustment.

Potential exposure to blood, body fluids, and other materials; hazardous substances; needles, sharps, fumes, chemicals, humidity, cold, heat, adverse weather, animals, second‑hand smoke or vape; unpredictable home environments.

Safety requirements: closed‑toe shoes, facemask, goggles, gown, gloves, and functioning communication device for routine and emergency situations.

Travel Requirements

Willingness to travel up to 100% of the time for business purposes.

Preferred Qualifications & Interests

Bachelor of Science in Nursing from an accredited school of nursing preferred.

1–2 years of previous infusion nursing experience preferred.

Pay Range
Salary to be determined by the applicant’s education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay Range is $40.78–$67.97.
