We are seeking a compassionate, detail-oriented Case Manager to join our growing behavioral health and substance use disorder treatment team. The Case Manager plays a vital role in supporting clients throughout their recovery journey by coordinating care, completing assessments and documentation, ensuring compliance with regulatory standards, and serving as a liaison between clients, providers, and external agencies.
The ideal candidate is organized, empathetic, professional, and experienced in substance use treatment environments. This role requires strong documentation skills, effective communication, and the ability to work collaboratively in a fast-paced clinical setting.
Key Responsibilities
Client Care & Case Management
Conduct client intakes and complete required intake documentation accurately and timely.
Complete and maintain ASAM assessments, treatment plans, and progress notes.
Provide case management services to individuals with Substance Use Disorders (SUD).
Coordinate referrals and provide appropriate community and recovery resources.
Maintain consistent communication with clients to support treatment engagement and retention.
Assist clients with navigating treatment services, insurance, and support systems.
Documentation & Compliance
Maintain accurate, timely, and compliant client records in accordance with:
The Joint Commission standards
COMAR 14.31.06
HIPAA regulations
Ensure documentation is complete, legible, and submitted within required timeframes.
Conduct and document urinalysis testing per program protocols.
Utilize Zoobooks and/or electronic systems for documentation and tracking.
Care Coordination
Communicate and coordinate with Carelon and other insurance providers regarding authorizations and treatment updates.
Collaborate with clinical staff, counselors, nurses, and leadership to ensure continuity of care.
Participate in team meetings, case conferences, and treatment planning.
Qualifications
Bachelor’s degree in Human Services, Social Work, Psychology, or related field (preferred)
Experience working with substance use disorder populations preferred.
Strong organizational, communication, and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Proficiency with electronic health records and documentation systems.
Ability to maintain professionalism, boundaries, and confidentiality.
