Job Description
AHMG Pediatric Gastroenterology at Orlando
615 E. Princeton St., Suite 225, Orlando, FL 32803
Monday - Thursday 8am - 5pm, Friday 8am - 12pm
* may need to support offices in Lake Mary, Winter Garden, Celebration and Kissimmee as needed
Responsibilities
Identifies primary and/or secondary payer of services.
Participates in establishing the plan of treatment through communications with facilities discharge planning and/or patient’s physician.
Explains the function of the health team members who will implement the plan of treatment to the patient.
Coordinates the start of care activities with the Intake Coordinator. Functions as an expert resource for interpreting Medicare home health guidelines.
Communicates pertinent information and instructions for specific physician’s orders to health team members, including completion of the interdisciplinary referral.
Provides information to physicians and the community on the benefits and alternatives home health services can provide.
Participates in and attends appropriate staff and committee meetings as directed by the Clinical Intake Manager or designee.
Documents patient care coordinating activities according to department procedures.
Makes appropriate referrals for patients not accepted for care. Performs other duties as assigned.
Responds to requests for care from referring sources within the community in a timely manner.
Obtains information to verify insurance coverage and informs patients.
Knowledge, Skills, and Abilities
Demonstrate knowledge of community resources and the medical community [Required]
Extensive clinical knowledge and evaluation skills with experience in developing patient care plans [Required]
Demonstrate understanding of the home health care delivery system and patient eligibility [Required]
Education
Associate's of Nursing [Required]
Bachelor's of Nursing [Preferred]
Field of Study

Work Experience
1+ nursing experience in home health. [Required]
