Position Overview
We are seeking a highly motivated and compassionate
Care Coordinator
who can wear multiple hats-managing caregiver schedules while actively seeking and onboarding new clients. This is a key role for someone passionate about home care, committed to ensuring our clients receive the highest standard of support and service.
Key Responsibilities
Coordination
Match clients with appropriate caregivers, ensuring availability, skill alignment, and compatibility
Maintain and manage caregiver schedules, including adjustments for changes, call-offs, and coverage gaps
Ensure compliance with agency procedures and Illinois non-medical home care regulations
Sales & Client Acquisition
Actively seek new clients through outreach, community engagement, and partnerships
Conduct intake consultations and present service offerings to prospective clients and families
Build and maintain strong relationships with hospitals, senior living communities, clinics, and local organizations
Track leads, conversions, and monitor client satisfaction
Qualifications
High school diploma or equivalent required; associate or bachelor's degree preferred
Minimum 2 years of experience in caregiving, scheduling, or coordination within a home care or healthcare setting
Previous experience in sales, client intake, or community outreach highly preferred
Familiarity with Illinois non-medical home care regulations
Strong written and verbal communication skills
Tech-savvy; proficient in scheduling platforms and mobile applications
Reliable transportation, valid driver's license, and proof of insurance (local travel required)
