Job Description:
Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals.
Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space.
Communicates through various digital channels to support patient care. Performs initial health screens and intake for new members.
Obtains and reviews old medical records to ensure comprehensive patient history. Identifies care gaps and orders necessary screening tests.
Confirms appointments and provides instructions to patients.
Knowledge, Skills, and Abilities:
Knowledge of chronic disease management [Required]
Empathetic nature and a distinct ability to consistently provide top tier service. [Required]
Driven by passionate desire to enhance and improve member outcomes. [Required]
Knowledgeable in member advocacy and navigating complex systems. [Required]
Responsible for managing the member's entire care plan with the guidance of the RN Case Manager and Provider. [Required]
Education:
Technical/Vocational School [Required]
Field of Study: Graduate of a Medical Assistant Program
Work Experience: N/A
Licenses and Certifications:
Basic Life Support
CPR Cert (BLS) [Required]
Certified Medical Assistant (CMA) [Required] OR
Clinical Medical Assistant Certification (CMAC) [Required] OR
Registered Medical Assistant (RMA) [Required] OR
Certified Clinical Medical Assistant (CCMA) [Required] OR
National Certified Medical Assistant (NCMA) [Required] OR
