Two assisted living buildings serving 33 Villagers
Eight Personal Care Homes (independent, semi-independent, and cottages) serving up to 100 Villagers
A 32-bed skilled nursing facilitythe only one in Georgia dedicated solely to adults with developmental disabilities
A Medicaid waiver day program with approximately 33 participants
A community-based independent living program, All In, currently serving 26 individuals

Position Purpose - Responsible for assisting in person-centered implementation of Individual Service Plan goals which includes community, vocational, life skills, and social skills training for villagers in the Community Access Program.
Required Qualifications
High School Diploma or GED with 2 years of experience with special populations desired.
Ability to navigate internet based programs.
Must be at least 18 years of age, have a valid Class C Georgia Driver's License, proof of vehicle insurance, and meet company policy regarding MVR requirements.
Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.
Must undergo and pass a drug screening and screening for Tuberculosis.
Major Duties and Responsibilities
Responsible for implementing and documenting ISP goals including the frequency of the individual goal, vocational skills training, arts/academics, leisure, and recreation programs with the Villagers to meet individual, on a daily basis.
Track and document items for HRST, behavior and health and safety.
Monitors onsite and offsite meals, field trips and special events as required.
Responsible for the implementation of community activities to ensure compliance with villager goals.
Responsible for meeting all regulatory requirements and abiding by Annandale and Department policies by completing all Medicaid required documentation in a timely manner, as well as adhering to HIPAA practice policy and procedures through the use of an electronic record software system.
Provides Transportation for villagers as assigned by Program Coordinator.
Monitors Villager behavior and assists in developing positive behavior.
Perform daily hygiene checks and provide hygiene assistance.
Provide feedback for Individual Service Plan meetings for Villager's, evaluating and giving input regarding Villagers' progress and goal setting as needed.
Shares information on skills attainment, behavioral problems, etc. for ISP input and daily goal tracking.
Assists with upkeep of equipment, supplies and other operating materials.
Assists in keeping facility clean according to Jackson House cleaning policy.
Assists with Saturday programming as assigned.
Assists with planning and implementing special events, parties and field trips.
Attends all required and assigned trainings, to include online training required by DBHDD and Annandale Village as they are assigned.
Performance will be reviewed on a weekly basis by Program Coordinator, to include reviewing daily documentation and yearly using Annandale's yearly performance review form.
Performs other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care of the Villagers.

Ability to read, write, and speak English.

While performing the duties of this job, the employee is regularly required to use hands; use fingers; handle or feel; reach with hands and arms; talk; and hear.
The employee frequently is required to sit, stand, and walk; climb or balance; stoop, kneel, crouch, bend, or crawl. Employee is required to lift up to 25 pounds and/or move up to 75 pounds (with assistance).
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
Employee is required to transport (drive) and assist Villagers in and out of vehicles.
Employee is required to perform CPR/First Aid.
FT (40) Monday - Friday (7:30 am- 4:00 pm) working one Saturday monthly (4 hour shift).
