What You'll Do
Deliver compassionate, high-quality care to patients across the lifespan
Partner with providers and care teams to ensure seamless, coordinated treatment
Support patients and families in understanding health needs and next steps
Document care accurately using our EHR system
Participate in care huddles and team-based quality improvement initiatives
Serve as a key member of our Patient-Centered Health Home model
What We're Looking For
Required:
RN / LPN / CMA License OR Certification, active and in good standing with the State of North Carolina
Graduate of an accredited program
Current CPR/BLS certification
Valid Driver's License
Preferred:
One (1) year of clinical experience in a primary care, community health, or outpatient setting
Experience with underserved or rural populations
Familiarity with team-based care or PCMH/PCHH models
