Key Responsibilities
Perform in-home PT evaluations and create personalized care plans
Provide skilled PT interventions to enhance strength, balance, mobility, and safety
Educate clients and families on exercises and home safety
Collaborate with a multidisciplinary care team
Document care using mobile-friendly EMR
Qualifications
Active MA PT license
DPT or MPT from an accredited program
CPR certified
1+ year of experience preferred (new grads welcome)
Reliable vehicle & valid driver’s license
