Job Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team. The LPN plays a critical role in delivering high-quality care to residents by supporting daily nursing operations, coordinating with interdisciplinary teams, and ensuring each resident's individualized care plan is followed with excellence and empathy.
Key Responsibilities:
Provide direct nursing care to residents in accordance with their care plans
Administer medications and treatments as prescribed by physicians
Monitor residents conditions and report any changes to the RN or physician
Document all nursing interventions and resident responses accurately and promptly
Assist with assessments, admissions, discharges, and transfers of residents
Collaborate with CNAs and other staff to ensure the highest quality of care
Maintain a clean, safe, and supportive environment for residents
Provide education to residents and families regarding health conditions and care needs
Uphold all state, federal, and facility regulations and standards of care
Participate in ongoing training and quality improvement initiatives
Qualifications:
Current and valid LPN license in the state of Minnesota.
Graduation from an accredited practical nursing program
CPR/BLS certification
Long-term care or skilled nursing experience preferred
Strong clinical, communication, and critical thinking skills
Compassionate, reliable, and team-oriented attitude
