Elios, Leominster, MA, United States

Nurse Practitioner or Physician Assistant – Primary Care / Family Medicine

Full-Time

Are you a primary care NP or PA who wants more than a packed panel and a productivity quota?
We're a community health organization in central Massachusetts looking for a passionate, patient-centered Nurse Practitioner or Physician Assistant to join our primary care team. If you want to practice the kind of medicine you went into healthcare for — with real support, a genuinely collaborative team, and a workplace that treats its people well — keep reading.

What Makes This Role Different
This isn't a revolving-door practice. Our providers stay because the mission is real, the team is strong, and the organization actually invests in the people who show up every day.

What You'll Do
As a primary care NP or PA, you'll manage a diverse patient panel across the full spectrum of family medicine — acute visits, chronic disease management, preventive care, and care coordination. You'll work alongside a multidisciplinary team with real ancillary support so you can focus on what you do best: taking care of patients.
Manage acute and chronic conditions across all age groups
Conduct wellness exams, preventive screenings, and immunizations
Collaborate with behavioral health, care management, and specialty teams
Prescribe and manage medications with appropriate follow-up
Provide patient education and support self-management
Maintain accurate, timely documentation in the EHR

Why Providers Choose Us
A workplace that recognizes you.

From birthday gift cards to event ticket giveaways, staff appreciation isn't an afterthought here — it's baked into the culture.
An on-site café you'll actually use.

A beloved local café operates right on-site, serving breakfast and lunch Monday through Friday. No sad vending machine lunches.
Fresh local produce, steps from your office.

A mobile produce truck from a local farm partner stops at our main location weekly from May through November. Fresh, local, and convenient.
Local art in your workplace.

Through a partnership with a regional art museum, our sites feature rotating exhibitions of local artwork — because a beautiful environment matters for staff and patients alike.
Stay active without leaving work.

An on-site exercise room with treadmills, ellipticals, and bikes is available for staff use. And if you prefer the outdoors, conservation land and reservoir walking paths are right outside the door.
A community that has fun together.

Annual Halloween costume and site decoration competitions bring the whole team together in the best way. Friendly rivalry included.
Easy to get here — however you commute.

A commuter rail station is approximately one mile away, and regional transit buses serve all of our sites — making us one of the most accessible employers in the area.
