Job Title: Integrated Case Manager
Location: Detroit
Duration: 13-week contract (possible extension)
Shift: 8:00 AM - 4:30 PM (weekend rotation)
Pay Rate: $57/hour
Duties:
Conduct patient assessments (clinical, social, and functional needs)
Develop and manage individualized care plans
Coordinate care and ensure smooth transitions across settings
Collaborate with multidisciplinary teams for complex cases
Educate and support patients and families
Monitor patient progress and adjust care plans as needed
Requirements:
5+ years of case management experience with 2+ years recent acute inpatient experience
Current MI RN licensure or LMSW
Discharge planning experience
