The Medical Social Worker (MSW) is responsible for supporting patients and families through care coordination, discharge planning, and psychosocial assessments in an acute care setting.
This role requires strong clinical judgment and experience managing patient caseloads.
Must-Have Qualifications
•
Master’s Degree in Social Work (MSW) – Required
• Minimum
1+ year of acute care hospital experience
•
Travel experience required (No first-time travelers – HARD STOP)
• Strong knowledge of
care coordination and discharge planning
• Ability to manage
caseload of 20–23 patients
