Description
Care Navigation plays a critical role in helping individuals access the right support at the right time across complex healthcare needs. This program brings together a multidisciplinary team of pharmacists, social workers, nurses, dietitians, and care coordinators to guide members through the often fragmented U.S. healthcare system.
As a Care Coordinator, you will support members dealing with chronic or serious conditions by helping them better understand their care options, navigate benefits, and coordinate services. You will report to the Care Coordinator Team Lead and work closely with care managers, providers, and internal teams to deliver high-quality, cost-effective care support.
Responsibilities
• Provide telephonic support to members, care managers, and the Care Navigation program
• Own and manage non-clinical follow-ups for assigned cases, including benefits and claims research, provider research, and medical record retrieval with a focus on cost savings
• Coordinate directly with members, providers, and vendors to ensure seamless care delivery
• Co-manage cases with care managers, ensuring timely completion of all assigned tasks
• Act as a liaison between Care Navigation and internal Customer Service teams to ensure accurate and timely resolution of member needs
• Utilize multiple systems (including Google Suite) to troubleshoot, respond to, and document member inquiries
• Support additional projects and responsibilities as assigned
Basic Qualifications
• 1+ years of experience in a customer-facing role (healthcare experience preferred)
• Strong ability to manage multiple priorities, stay organized, and meet deadlines
• Excellent verbal and written communication skills
• Ability to handle high-volume workloads with professionalism, empathy, and composure
• Comfortable navigating multiple systems and applications with efficiency
Preferred Qualifications
• Bilingual (English/Spanish) is a plus
• Associate’s or Bachelor’s degree is a plus
